ACL reconstruction in skeletally immature patients present unique challenges secondary to potential for physeal injury. There has been much debate about the most appropriate graft choice in this population but there seems to be agreement that bone blocks should not be used across open physes. The quadriceps tendon has shown good results as an effective graft choice but since the classic quadriceps tendon includes patellar bone block, this would be excluded from consideration in patients with open tibial and femoral physes. This video shows graft prep technique for all-soft tissue quadriceps tendon autograft that can be used in all populations.
